Welcome to Merrimack Perio & Implants PLLC. This is our practice’s policy regarding SMS/Text Communication method. During part of our patient intake process, there’s an option to select how we may contact you in accordance with our Notice of Privacy Practices. By opting into SMS communications from us, you agree to the following Terms of Service:

Types of Messages


By subscribing to our SMS services, you will receive SMS messages related to the following:

  • Pre-Op and Post-Op care instructions and direct communication with our periodontist, Dr. Howard Yen, in regard to any clinically relevant matters.

  • Appointment reminders and confirmations

  • Treatment or service reminders

  • Important office announcements or closures

  • Payment and Billing Reminders: notifications related to billing, including payment reminders, payment confirmations, payment links, and receipts for services rendered.

Messaging Frequency

We will send SMS/Text messages to you based on the following schedule:

  • Pre-Op and Post-Op period: Varies and as needed

  • Appointment reminders: 2 weeks, 5 days & 2 days before your appointment

  • Other relevant office operation updates (such as office hours): Varies and as needed

Messaging and Data Rates


Standard message and data rates may apply from your mobile carrier. Please check with your mobile carrier for additional charges that may apply.

Privacy & Data Usage


Your phone number and other Protected Health Information (PHI) will be handled in accordance with our practice’s Privacy Practices. We do not sell, rent, or share your information with third parties without your prior consent. SMS consent is not shared with third parties.

Opt-out Instructions


You may opt-out of receiving communication via SMS/Text messages at any time by notifying our practice directly via phone call, e-mail, in-person or text STOP. and we shall update your communication preferences promptly. Changes to Terms/Policy
